SV DNA Kit Order Form

Please Note the Following When Submitting your DNA Kits for Processing!

There are 2 options. The DNA can be Stored only or the DNA can be Tested
- To have the dogs DNA Tested, both the sire and dam must have their DNA listed in SV-DOxS as Tested
- To have the dogs DNA Stored, neither the sire or dam needs to have Tested or Stored DNA in SV-DOxS
- The dog’s original pedigree must be mailed to the USCA office along with the DNA kit as the original pedigree will be sent to the SV to be stamped with the DNA information.
- We do not accept anything other than DNA kits. PLEASE DO NOT send vials of blood!
