USA Forms

This page contains the USA forms. These are saved in Adobe Acrobat PDF format. You can enter your information in the form and print it out. To save a blank copy of the form or document, right-click on the link and select the Save Target As … menu choice.

INSTRUCTIONS FOR PDF FILES

Many of the USA forms are formatted as PDF files. This will allow you to complete all forms on your computer, and then print the form.

Please go to the on-line form pages, select, complete and print the desired form. Once printed the form may be faxed or mailed directly to the USA office according to the instructions on each form.

You may also save the PDF files to your computer. To save a PDF file, right-click on the link, select "Save Target As …" and follow the download instructions that appear.

Forms in PDF format require Adobe Acrobat Reader^® to view. This is a FREE download from Adobe. If you do not have Acrobat Reader^®, you must download Acrobat Reader. Most computers will already have this program installed.

  All USCA Judges have been issued a copy of the answers . All answers can be found in the current rule book or on the USCA website under rules and regs.
  Regarding the new BH Tests and Foreign Judges
  
  If a club is hosting a trial using a Foreign Judge (IE: SV, GSSCC, FCI from a WUSV Organization) and the Judge is not familiar with our rules and our BH test, the Trial Secretary should be familiar with the test and have or know the correct answers to be able to assist the Judge with the administration of the test and grading of the completed tests.
  
  Trial Secretaries may contact the USA Director of Judges in advance of the trial for an answer sheet for the current test if needed. Click here to request the answer sheet.
- New IPO Judge's Sheets - January 1, 2012. The following score sheets reflect the rule changes for 2012. Discard any copies of the old score sheets and use these sheets for all events after 1/1/2012.

  Use these forms if you want to get your dog a dual USA/SV registration. For these forms, you will need to submit the ORIGINAL pedigree and certificate. The SV also requires that certain forms be completed.
  - USA/SV Individual Registration
  - USA/SV Litter Registration
- USA-ONLY registration
  Use this form if you want to get your dog registered in the USA registry. This requires COPIES of the dog's papers. This is a different form than the USA/SV form.
  - USA-ONLY Individual Dog Registration.

- X-Rays for SV HD/ED evaluations
  
  ***EFFECTIVE IMMEDIATELY***
  
  If submitting x-rays to the USCA office for processing of the HD/ED evaluations, a letter from your veterinarian on his/her letterhead stating the following, is now required.
  1. Confirmation from the veterinarian that he/she verified the identity of the dog presented to them by means of the tattoo and/or microchip number, dog’s full registered name, and registration number.
  2. Notation from the veterinarian stating whether or not he/she sedated the dog for the x-ray procedure.
  3. Authorization (signatures) from the owner/co-owner(s) that all information within the identification plate AND within the veterinarian’s letter has met their approval to be forwarded onto the SV office in Germany for processing.
